This, he said, made it difficult to implement consistent change.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">Away from the inquiry, in October 2024, the health secretary stated that the government intended to reform the regulatory system.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">This was in response to a review of the way the Care Quality Commission (CQC) inspected trusts, called the Penny Dash Review, which said the framework was too complicated.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">The Lampard Inquiry will consider the CQC&#8217;s role in relation to events in Essex.<\/p>\n<\/div>\n<h2 class=\"sc-f98b1ad2-0 eOFjmw\">Analysis<\/h2>\n<div data-component=\"text-block\" class=\"sc-3b6b161a-0 dEGcKf\">\n<p class=\"sc-9a00e533-0 hxuGS\">Three systemic issues raised by the inquiry &#8211; staff shortages, poor data, and regulatory complexity &#8211; have been longstanding concerns. <\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">The <a target=\"_blank\" href=\"https:\/\/www.rcn.org.uk\/news-and-events\/news\/RCN-responds-to-HSSIB-mental-health-report-24102024\" class=\"sc-f9178328-0 bGFWdi\">Royal College of Nursing<\/a>, the <a target=\"_blank\" href=\"https:\/\/www.cqc.org.uk\/publications\/monitoring-mental-health-act\/2022-2023\" class=\"sc-f9178328-0 bGFWdi\">CQC<\/a> and a <a target=\"_blank\" href=\"https:\/\/committees.parliament.uk\/committee\/127\/public-accounts-committee\/news\/196555\/concerning-pressures-on-nhs-mental-health-staff-causing-vicious-cycle-of-staff-shortages\" class=\"sc-f9178328-0 bGFWdi\">2023 Public Accounts Committee report<\/a> all flagged staffing shortages and burnout. <\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">A 2023 review found Norfolk and Suffolk NHS Foundation Trust had <a target=\"_self\" href=\"https:\/\/www.bbc.co.uk\/news\/uk-england-norfolk-65962463\" class=\"sc-f9178328-0 bGFWdi\">lost track of patient death data<\/a>, while a 2025 <a target=\"_blank\" href=\"https:\/\/www.hssib.org.uk\/patient-safety-investigations\/mental-health-inpatient-settings\/fourth-investigation-report\/#findings\" class=\"sc-f9178328-0 bGFWdi\">Health Services Safety Investigations Body (HSSIB) <\/a>report called for a unified national dataset. <\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">Regulatory reform is also under way following multiple critical reviews. <\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">While Baroness Lampard is expected to reference these reports, the inquiry is also under pressure to uncover new evidence.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">Some families have expressed concern regarding its pace, and limited focus so far on cultural change.<\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">They have also noted that safeguarding issues, such as patients absconding from units, have received little attention &#8211; a relevant issue given a recent inquest into the death of an 18-year-old who died while on escorted leave from an EPUT unit.<\/p>\n<\/div>\n<h2 class=\"sc-f98b1ad2-0 eOFjmw\">Transparency and whistleblowing<\/h2>\n<div data-component=\"text-block\" class=\"sc-3b6b161a-0 dEGcKf\">\n<p class=\"sc-9a00e533-0 hxuGS\">Only 11 out of 14,000 staff came forward during the earlier non-statutory phase of the inquiry. <\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">Baroness Lampard has said she will use statutory powers to compel evidence if necessary. <\/p>\n<p class=\"sc-9a00e533-0 hxuGS\">Mr Scott acknowledged that &#8220;closed cultures&#8221; During a recent inquest into the death of a 16-year-old patient, a manager testified that staff were reluctant to raise safety concerns.
